It's because their faces were painted so white that in contrast their teeth looked yellow and rotten; they couldn't whiten their teeth at the time, so instead they just painted all of them black. Weird, right? It looks totally creepy when they do it in movies.

Now that you say this...I realize I have never seen a picture of a Geisha smiling except in current photographs.
